Special Education Forms for Faculty
Input from teachers is essential to the creation of the Individual Education Program (IEP) for students. The input from general education teachers is directly connected to the Present Level of Performance (PLP) on the IEP. Teachers will be contacted by case managers and asked to complete this form. Please download and either type responses or handwrite them; please deliver hard-copy only to case managers or Rachele Preston, CSE/CPSE Secretary.Teachers and staff trained in Nonviolent Crisis Intervention (CPI) should complete this form within 24 hours and submit to the Director of Pupil Services when any strategies are utilized.
