Teacher Input Questionnaire
Input from teachers is essential to the creation of the Individual Education Program (IEP) for students. The input from general education teachers is directly connected to the Present Level of Performance (PLP) on the IEP. Teachers will be contacted by case managers and asked to complete this form. Please download and either type responses or handwrite them; please deliver hard-copy only to case managers or Rachele Preston, CSE/CPSE Secretary.- Teacher Input Questionnaire (Word document)

Physical Intervention Form
Teachers and staff trained in Nonviolent Crisis Intervention (CPI) should complete this form within 24 hours and submit to the Director of Pupil Services when any strategies are utilized.One-to-One Aide Needs Consideration Form
Special Education teachers seeking additional support for students in terms of 1:1 aide support should complete the following form:Referral Forms
Referrals to the CSE Committee from the RtI Committee and Building Principals should complete the following forms:Referrals to the 504 Committee should be made by completing the forms below:Teachers are encouraged to communicate concerns about students to parents. They will complete this form and submit to the CPSE Office who will then mail home to parents:
Chapter 408 Acknowledgment Form
All faculty and staff members are required to sign-off on this form upon review of a student’s 504 Plan or IEP. This signature will serve as acknowledgment of the IEP or 504 Plan and the implementation responsibilities.
